The service exists for frail and/or disabled members of the community who are finding it difficult to function as well as they used to, for whatever reason. Clients do not have to be elderly to receive support from the therapy team - the skills of caring are available for younger disabled persons - from 16 upwards.
The Reablement Team, which is based in Holsworthy Hospital, is a friendly group of healthcare professionals. Referrals can come from anyone who needs help for themselves or for someone they know. Referrals can also be made by relatives, friends, carers, the clients GP.
After an initial assessment in the clients usual environment, active therapy, advice and support can be provided either in the unit, in their own home or wherever the client is living at the time. Transport to the unit may be available and a lunch can be provided, if necessary, for which there is no charge. Persons referred to reablement will not necessarily be seen by all the healthcare professionals in the team - only those it is appropriate for them to see.
The Reablement approach is a team approach and the local GP practise will be kept informed of the clients progress.
